NCF: “Harness the expertise of local care providers”

System leaders need to harness the expertise of local care providers to ensure the right care and support is available to people with complex care needs, a National Care Forum round table has heard.

The discussion heard that shifts in condition complexity must be better recognised. Leaders said: “Conversations about discharge are often too overly focused on getting people out of hospital, now, and are not often enough about long-term planning.”

Consequently, some providers have found specialist recruitment difficult. Enhanced care in care homes can also be hampered by too narrow a focus on supporting discharge, and inadequate funding, the round table heard.

Summarising two things that need to continue, the round table cited:
• Improvements made since the inception of ICSs should continue to develop across the country
• Improved data infrastructure.

Of the things that need to stop, the round table highlighted:
• Short-term emergency discharge funding
• Categorising care.
