DHSC sets out vision for more NHS support for care homes

Better NHS support to care homes and improved data sharing are among the key objectives of a new health and social care integration plan in England.

A new Integration White Paper sets out how better joined-up services can support patient care.

More specialist support for care homes aims to reduce the number of hospital admissions, while improved NHS data sharing will allow the NHS to notify a local authority straight away if a person requires social care support.

Other improvements set out in the White Paper include:

  • Better transparency and choice: local populations will be able to see how their areas’ health and care services are performing and make decisions about their own care
  • More personalised care: wider community support – such as social prescribing – could reduce the need for more expensive, invasive medical treatment
  • Clear communication: a single digital care record to book appointments, order prescriptions, and communicate with care providers on one platform
  • Coordinated services: better integration across primary, community and hospital services and social care
  • More flexible services: aligned financial incentives and pooled budgets
  • Better value for money: reducing duplication and waste.
